Please access the consent form via the link below, complete it and upload with your application form below to enable us to complete the request within 3 working days.
You will also need to upload a photograph or scan of your identification (Passport (NZ or Int), Drivers or Firearms License Only).
Once you've uploaded the documents and submitted your application, we will review the information. If we have the necessary information you will receive an invoice for $55+GST .
On receipt of payment, the history request will commence.
